Sofwave and Ultherapy are both non-surgical, energy-based treatments that use ultrasound to stimulate collagen and tighten the skin. While they share a common goal, the way each technology delivers energyāand the results, comfort, and safety profileācan differ significantly. Not sure which treatment is right for you? How it works

Clinical studies for Ultherapy began as early as 1998, and the treatment became commercially available in the US in 2010. It has undergone extensive clinical studies and market evaluation for its safety and efficacy over the years. Ultherapy uses high intensity focused ultrasound (“HIFU”) to heat deep layers of our skin which triggers a natural healing process that creates new collagen and elastin. Regeneration of the collagen and elastin helps firm, tighten, and lift our skin.
Sofwave is considered a next-generation skin tightening treatment that also uses ultrasound energy. Compared to Ultherapy that uses focusedĀ ultrasound energy to target very precise areas, Sofwave uses multiple simultaneous parallel beams to deliver ultrasound energy to ensure broader coverage in the mid-dermis.
In terms of treatment depth, HIFU treatments such as Ultherapy reach a lot deeper into our skin. Ultherapy can reach 1.5mm, 3.0mm, and 4.5mm into our skin, which includes the SMAS layer (“Superficial Musculo-Aponeurotic System”). Sofwave precisely targets the mid-dermis, 1.5mm below the surface of our skin, which is where the majority of our collagen and elastin lie. Deeper treatment depths allow Ultherapy to lift and tighten the muscle layer but may involve more discomfort and a higher risk of fat loss if not administered properly. Sofwave’s treatment depth is more superficial which helps address skin laxity where it matters the most and makes it more tolerable for patients.
Sofwave also integrates a cooling technology to cool the surface of our skin while heating the dermis to stimulate collagen and elastin. This cooling technology not only enhances patient comfort during the treatment but also helps protects the skin from risks like burns or post-inflammatory hyperpigmentation.
Results
Both Ultherapy and Sofwave are FDA cleared for treating mild to moderate skin laxity. Sofwave is FDA cleared to:
- Tighten and lift the eyebrows, neck, and jawline
- Improve mild skin laxity in the upper arms
- Reduce the appearance of fine lines & wrinkles
- Improve the appearance of cellulite
- Treat acne scars
Because Ultherapy reaches deeper into our skin, it is generally considered to offer a deeper lifting effect but with higher discomfort. Sofwave is optimized with precise depth and temperature to deliver tightening and lifting results with lower discomfort and downtime.
Ideal Candidate
Results for skin tightening devices are generally a lot more subtle compared to facelifts which surgically lifts our skin. Surgical facelifts provide more dramatic results in just one procedure, but come with much longer recovery periods with an average cost ranging from $20k to $50k. Surgical facelifts also primarily help with sagginess by lifting the skin, but do not improve skin tone, texture, or elasticity the way non-surgical collagen-stimulating treatments can.
Ultherapy and Sofwave treatments work by stimulating and creating new collagen and elastin which improve the overall quality of our skin. They are ideal for those in their 30s to 50s with mild to moderate laxity who are looking for powerful skin rejuvenation with little to no downtime. With consistent treatments, they help delay the skin’s aging process over time and reduce the need for a surgical procedure. Older patients may also choose these treatments to postpone the facelifts or combine them with facelifts to enhance the results.
Downtime
Both treatments allow patients to return to normal activities immediately. However, Sofwave is generally associated with less post-treatment swelling, redness, or tenderness than Ultherapy.
Treatment Sessions
For mild to moderate laxity, both treatments only require one session. For severe laxity, you may need additional sessions for optimal results. Maintenance treatments may be recommended once a year for both treatments, depending on your skin goals and the treatment plan discussed during your consultation.
Safety
Both treatments are FDA cleared and considered generally safe for skin tightening and rejuvenation. However, because Ultherapy reaches deeper layers, there is a slightly higher risk of complications such as unintended fat loss or nerve sensitivity if not administered with proper technique. These risks are rare and greatly reduced when performed by experienced providers.
Sofwave’s treatment depth and temperature settings are also pre-set to be precisely at 1.5mm deep and 60 – 70 degrees Celsius. There is significantly lower risk of fat loss or nerve damage with Sofwave compared to Ultherapy.
Patient Comfort
Ultherapy is generally considered more uncomfortable due to the focused, deeper energy delivery. Sofwave, especially when used with a Zimmer cooling device and Pro-Nox (laughing gas), is typically well tolerated. Most patients describe the sensation as āwarm zapsā that are manageable without numbing.. At U Med Spa, we provide complimentary Pro-nox (laughing gas) and Zimmer for Sofwave treatments.
Final Note
Results from any laser or energy-based treatment depend on how your body responds to controlled injury. These treatments trigger your skinās natural healing process by creating thermal stimulation, stimulating the natural regeneration of collagen and elastin. Your immune health, age, and baseline skin condition can affect results. A healthy lifestyle and proper skin careāboth before and after treatmentācan significantly enhance your results.
